Location: Mosaic Primary Care Network (NE/SE Calgary)
Employee Class: Regular Full Time
FTE: 1.00
Salary: Rate of pay to commensurate with qualifications and experience
Summary:
This Social Work Clinician works with an interdisciplinary team serving community-based clients within the Mosaic Primary Care Network. The Social Work Clinician will be responsible for developing a program of Social Work assessment, treatment, and consultation services for clients and ensures that Social Work clinical practice is evidence based and meets professional standards of practice. The position involves provision of patient care, group patient education, consultation to staff and clients, orientation of new staff and students to the service, participation in staff development, and participation in program development and evaluation.
Responsibilities:

Provides advanced level counseling and intervention which includes counseling in the following areas: crisis intervention; addictions; decision-making around treatment modalities; grief/bereavement, catastrophic illness, new diagnosis, chronic illness, illness adjustment; care giver support; conflict resolution; safety planning; resource; psychotherapy/brief therapy; palliative & end-of-life

Provides advanced level assessment including assessment of patient/family strengths and needs, patient/family’s history of medical, social, psychological, spiritual and psycho-social dimensions; ability to assess for mental status, suicide risk, and coping abilities

Screens for domestic violence and other abuse such as partner, elder, child, substance abuse and sexual abuse; and intervenes in an appropriate manner (ie safety planning, contacting child welfare if children present)

Works directly with and/or provides consultation to the team on complex cases requiring advocacy, mediation and ethical consultation

Participates in patient conferences to communicate within the interdisciplinary team, and records appropriately in the patient record

Communicates with various members of the team as to any changes pertinent to either the patient or their family’s status

Provides leadership on multidisciplinary teams when complex psychosocial issues impact patient care

Advocates for culturally competent health care within the community

Identifies and facilitates community supports, outreach and liaison

Provides in depth knowledge of a range of specialized resources that affect patient care, for example, basic needs (food, shelter, clothing, transportation, etc.) and specialized resources, (financial resources such as pensions, AISH, disability insurance)

Provides liaison to community agencies and other referral agencies

Responsible for the identification of need for and creation of disease specific education such as stress management, anxiety, and life skills teaching
Qualifications:

MSW from accredited university required

Registration as a social worker with the Alberta College of Social Workers required

Demonstrated clinical expertise and competence in dealing with health care issues & addictions

Experience working in a collaborative, consultative manner with a varied interdisciplinary team

Experience with adult education and group facilitation preferred

Recent Experience and advanced skill in crisis intervention/family therapy, psychotherapy/counseling with patients and families required

In-depth knowledge and experience in working with community resources related to chronic disease, Income Security programs at community/provincial/federal levels.

Must possess strong participative and collaborative skills including excellent communication and consultation skills

Must demonstrate flexibility and an ability to provide service within a continually changing health care environment

Experience working with an adult chronic disease population and medical social work preferred

Knowledge of a second language or working with diverse populations is an asset, but is not necessarily required

Must demonstrate initiative and sound judgment
