Job Summary:

The Calgary Mosaic Primary Care Network is an arrangement between a group of family physicians and Alberta Health Services - Calgary to provide primary care services to patients living in Northeast and Southeast Calgary.

 

The Calgary Mosaic Primary Care Network (MPCN) is seeking an experienced Pharmacist to work in a community-based multidisciplinary clinic and alongside family physicians.  The pharmacist will work in a team based environment which will focus on providing clinical pharmacy services to meet the Mosaic PCN objectives.

 

Duties include:

·       Working closely with all medical professionals, including family physicians in determining, monitoring and managing drug therapy for patients with complex, chronic diseases

·       Complete structured medication reviews for patients in our clinics to identify, prevent and resolve drug related problems

·       Educate patients and caregivers regarding the safe and appropriate use of drugs, and the importance of complying with drug therapy instructions

·       Assist patients, caregivers, and other health care providers by providing advice regarding the selection and use of drug and non-drug products

·       Adapt prescriptions regarding dose and dosage form to meet individual patient needs in collaboration with the physician

·       Assist patients in improving adherence to medications

·       Provision of drug information to patients, caregivers and multidisciplinary health care team members

·       Participate in the development, implementation and evaluation of patient care programs within Mosaic Primary Care Network

 

Qualifications:

  • Computer skills
  • Bachelor’s degree in Pharmacy
  • Registered with the Alberta College of Pharmacists (ACP)
  • Additional certification or experience in a specialty area is an asset (ie. Certified Geriatric Pharmacist, Certified Asthma Educator, Certified Diabetes Educator, Women’s Health etc.)
  • Communication skills
