Job Summary:
The Calgary Mosaic Primary Care Network is an arrangement between a group of family physicians and Alberta Health Services - Calgary to provide primary care services to patients living in Northeast and Southeast Calgary.
The role of the Mosaic PCN Licensed Practical Nurse is to work collaboratively within the After Hours Clinic to enhance the delivery of primary care services to patients across all age groups. The nurse will work within scope of practice to participate in the provision of care to meet the needs of patients with a variety of clinical conditions.
The hours of operation for the Mosaic PCN After Hours Clinic are 6-10 pm Monday to Friday, 11-4 pm weekends and statutory holidays.
Duties and responsibilities:
· Work within scope of practice as part of the clinic team to provide support to physicians and patients in day-to-day clinical activities:
o Perform a variety of nursing duties to provide consistent high-quality care to meet the needs of the patients through the lifespan
o Cooperate in developing strategies for improving the provision of health screening and follow-up care for the practice population
o Initiate appropriate investigations, in accordance with physician protocols
· Assist patients to navigate through the health system:
o Communicate with patients to determine the next appropriate health services required, based on presenting concerns and test results
o Provide direction in accessing appropriate local and regional health programs
o Initiate appropriate investigations
· Ensure the maintenance of accurate records in line with practice policy
· Other duties as assigned to maintain seamless patient flow throughout office visits
Abilities/Knowledge and Skills:
· Knowledge of nursing processes and practices, including intervention strategies and therapies
· Strong, effective written and oral communication skills, high level of perceptiveness
· Critical thinking skills
· Ability to build good working relationships with healthcare professionals, clients and other members of the community
· Ability to prioritize, manage time effectively and be flexible in a very active work environment
· Respect and understanding of confidentiality
· Customer service orientation
· Ability to manage a complex changing environment
· High levels of integrity and professional behaviour
· Evidence of own professional development
Qualifications/Skills:
· Successful completion of an accredited Practical Nurse diploma program
· Current CLPNA registration, CPR and BCLS
· Five years or more of related nursing experience
· Primary Care delivery to all age groups
· Demonstrated and effective communication skills
· Willingness to work in a dynamic environment that re quires flexibility and balancing of competing priorities
· Valid driver’s license
Preferred Candidates will also possess:
· Successful completion of the following courses: Medication Administration / Intramuscular and Intradermal injection course & Immunization
· Clinical experience in primary care nursing, community, gerontology and/or chronic disease management
· Advanced skills in health promotion, health screening, assessment and education
· Experience in developing priorities with variable workloads
· Initiative, accountability, sound decision-making skills
· Experience working in a team setting with other health professionals
· Basic computer skills, ability to use Electronic Medical Records

tertiary, and long-term care through specialty care linkages to primary care
•Facilitate the greater use of multidisciplinary teams to provide comprehensive primary care
